| Citizenship Verification | Based upon the information you provided on the Free Application for Federal Student Aid (FAFSA), the Social Security Administration was unable to confirm your US citizenship. |
| Consortium Agreement | For students wanting to receive financial aid for classes they are enrolling in at another school. Please Note: classes must be required for your program at NTC to qualify. |
| Homeless/Unaccompanied Youth Verification | Use this form if you reported on your FAFSA that you were an unaccompanied youth who is homeless or are an unaccompanied youth providing for your own living expenses who is at risk of being homeless. |
| Identity and Statement of Educational Purpose | The U.S. Department of Education has implemented regulations, which requires some applicants to submit a Statement of Educational Purpose and a copy of a valid government issued photo identification (ID) such as a driver’s license, passport or other state-issued ID. |
| Parent Marital Status Change Request | Use this form if your parent (dependent student) had a change in marital status after filing your FAFSA and you would like our office to update your parent’s marital status to reevaluate your financial aid eligibility. |
| Parent Refusal to Provide Information | If you have a situation where your parent refuses to provide their information on the 2024-2025 FAFSA and refuses to provide you with financial support, please complete and submit this form. |
| Physician’s Certification and Borrower’s Acknowledgement of Obligation | Complete this form is you have had one or more student loans discharged because of a total and permanent disability. Before you can receive additional federal student loans, Federal regulations require that this form be completed and returned to the Financial Aid Office. |
| Student Marital Status Change Request | Use this form if you (the student) had a change in marital status after filing your FAFSA and you would like our office to update your marital status to reevaluate your financial aid eligibility. |

Wausau Campus (enter through Main Entrance and deliver to Student Services) or drop-off at any of NTC’s regional campus locations, leaving it with the regional campus front desk staff.
